Product Description
CenturyLink Choice Business (QCB) Prime is a package that includes a flat rated business line and a choice of 3 features. Additional "Add On" features are available, after the initial 3 feature selection has been made, at the tariff rate, minus any applicable Wholesale discount.
CenturyLink Business Voice Mail Service is not part of the QCB Prime package but is available at a discounted tariff rate, minus any applicable Wholesale discount.

Availability
The QCB Prime package is available where facilities exist throughout CenturyLink QC except in Idaho-North (ID-N) and in Rate Group 3 in Oregon (OR).
NOTE: QCB Prime will be available in Arizona on May 4, 2006.

Compatibility & Restrictions
For specific feature compatibility and restrictions, see the individual Resale Features Product Catalogs (PCATs).
The QCB Prime package can not be provided on the same line with other packages.
The QCB Prime package is compatible with business line Universal Service Order Codes (USOCs) 1FB, AFK, 1FA, AFV, HFB.
Pricing
Rate Structure
The QCB Prime package may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.
There is no charge to add or remove QCB Prime features. There is no charge to add a package or QCB Prime features to an existing line.
Rates
The QCB Prime package may have a recurring and nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.
Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.
Monthly rates for packages that exceed the 3 QCB Prime choices (chosen from the QCB Prime Features List) will be determined by the feature chosen. The most expensive 3 features will be included in the package. Additional "Add On" QCB Prime features will be billed at the monthly tariff rate minus any applicable Wholesale discount.
Features chosen which are not on the QCB Prime Features List will be billed at the monthly tariff rate minus any applicable Wholesale discount.
CenturyLink Business Voice Mail Service is not offered as a QCB Prime feature but is available at a discounted tariff rate, minus any applicable Wholesale discount.
QCB Prime is available as a month to month service or as a 12, 24, or 36 month term agreement. Additional information for QCB Prime term agreements can be found in Tariffs/Catalogs/Price Lists.
Termination Liability Assessment (TLA) may apply for early termination of contracted services, additional information can be found in Tariffs/Catalogs/Price Lists.
NOTE: The 12, 24, and 36 month term rates are currently not available in New Mexico.

The following ordering information applies when ordering QCB Prime on a 12 month term, 24 month term, or 36 month term:
A complete service request must be submitted and the order must be marked for manual handling including a Remark requesting the desired term, similar to the promotions ordering process. The start date of your desired term will be the same as the Completion Date of your order.
