The links on this page are provided to comply with the Louisiana Public Service Commission’s General Order in Docket No. R-26968, dated September 4, 2014. Pursuant to the Order CenturyLink makes available a Standard Pole Attachment Application, and a list of CenturyLink-approved contractors for pre-construction surveys and make-ready work associated with pole attachments in Louisiana.
NOTICE:
Prior to submitting an application for an attachment to a CenturyLink pole in Louisiana, it is necessary to obtain a Pole Attachment Agreement with CenturyLink. To request a Pole Attachment Agreement, please contact Poles@CenturyLink.com
Pursuant to the Louisiana Commission’s Order, CenturyLink is providing a Standard Pole Attachment Application via the link below. CenturyLink reserves the right to revise the Application from time to time. It is advisable to review this website prior to submitting an Application to ensure you are submitting the most current version.
In addition, pursuant to the Louisiana Commission’s Order, CenturyLink is providing a link to a page containing separate lists for CenturyLink-approved contractors for pre-construction surveys and make-ready work associated with pole attachments in Louisiana. The lists reflect contractors who are approved in CenturyLink’s procurement system when the lists are posted to this site. Although CenturyLink endeavors to keep the lists updated, the posting of the lists is not a representation, warranty or guarantee that a listed contractor is currently licensed or that a listed contractor is suitable for a particular job, nor is the posting of the list a representation of warranty of performance by any listed contractor. CenturyLink expressly disclaims and assumes no liability for any party’s choice of listed contractors or for any work performed by a listed contractor selected by any party.
